EMPOWER LIVES WITH CARE

Our mission at Open Hands Community Services is to promote independence, community involvement, and quality of life by providing effective, safe, caring, nurturing and developmentally appropriate person-centered services to those with developmental disabilities.

Person-Centered

All people with intellectual and developmental disabilities have strengths, abilities and inherent value. We work as a TEAM with the individual, those significant to their life, and DBHDS to provide tailored care that is focused on the individual's preferences and goals. We help in ways that makes best sense to them so they can lead fulfilling lives and achieve their dreams.

Skilled Caregivers

At Open Hands, we prioritize continuous staff training and education to ensure excellent quality of care. Trained in the Mandt system, our caregivers build healthy relationships, promote safe environments, and foster self determination for individuals. Our caregivers provide effective and safe care that respects the individual, and protects their dignity and rights while empowering them in their daily life. 

Skill Building

At Open Hands, skill building is not just educational—it’s transformational. We support individuals with developmental disabilities by helping them learn life skills so they become more self-sufficient, make informed decisions, and build meaningful relationships. As we support an individual's autonomy, we help build confidence and help individuals take ownership of their futures.

Planning

Successful support will stem from effective planning that has input from all team members, but directed by the individual receiving the services. We help develop Individualized Service Plans and Plans for Support that outline the person’s goals, interests, and needs, and serves as a roadmap for the support they will receive. We approach planning as an ongoing problem-solving process, used to help people with disabilities plan for their future. Our dynamic approach allows for flexibility, ensuring that our services adapt as the individual grows and changes.
